Application Steps

A participant may be referred by a human service agency or self-referred. The first step in the application process is to phone (319) 352-1108 or 1-800-410-7233 or use our Online Applicant Intake Form to provide general personal information. If the applicant is interested in continuing the application process, a personal/social history is completed. The Turning Point Project Administration team will determine participation eligibility based upon intake information and documentation.

An eligible applicant may be required to have one's name placed on a waiting list pending the availability of a transitional housing rental assistance opening.

Online Applicant Intake Form - We are currently in the process of updating our forms. Presently, you must have a valid e-mail server to submit our form to us. Please check back in late August for an improved, more user-friendly form that anyone can submit.

Directions: In a new browser window, you will need to complete the Intake Form. After completing the application questions and clicking the "submit" button once, close the window to return to the Cedar Valley Friends of the Family webpage. Please note that with our current form, the information you provide will NOT clear after clicking the submit button. After reviewing, a member of CVFF's Transitional Housing Team will contact you regarding any additional information needed or housing alternatives that may better suit your needs.
